You'll find it inside a small, dilapidated house. Look for a large, ornate wardrobe against the back wall of the main room. This wardrobe is illusory – interact with it and dispel the illusion to reveal the brain cylinder hidden behind it. Be ready for a fight, as dispelling illusions often attracts wylebeasts.
