Alright, so you've made it through the hospital and dealt with the Will o' Wisps. Now it's time to head into the main hospital area.
INTERLUDE: MANIACS
If you're playing the Maniacs version, things might seem a bit different. Instead of the elevator leading straight to the Wisps, you'll find yourself in a strange, new dimension. This is one of the first big changes you'll notice from the original game!
As soon as you step into this 'Strange Area,' you'll meet an old man in a wheelchair and his nurse. The old man will speak to you telepathically, asking you to 'show him your power.' Keep moving forward, and you'll soon have to face the Will o' Wisp fight. Before you do, though, check the corridor to the right – there's a chest containing 2 Medicines. Grab 'em!
Right after you defeat the Wisps, you'll find a Shinenai called a 'Dark Doctor.' This guy is super helpful because he'll heal you for free. Seriously, make full use of him, because there are a couple more challenging fights ahead.
The next encounter is with a Yuki Gaki, just a few steps from where you fought the Wisps. If you're playing on Hard mode, this fight can be brutal. The Gaki hits hard and can take you down in just two hits if you're unlucky, while it might take you three to defeat it. You might need to pop a healing item here. Once it's down, head back to the Dark Doctor to get fully healed up.
Continue through the door, and the old man in the wheelchair will reappear, gesturing for you to follow him. If you do, you'll end up in a battle against one Chirei Kodama and one Gedo Will o' Wisp. They can be a bit tricky, so don't hesitate to use healing items if they land a few good hits, but it's generally not too difficult.
Alternatively, you can go the opposite way from where the Wheelchair Guy went. You'll soon find yourself facing two Chirei Kodamas. These guys shouldn't give you too much trouble, and after you beat them, you can open a chest behind them containing a Maharagi Stone. This item is great for decimating the Kodama/Wisp fight you'd otherwise encounter down the corridor.
Regardless of which path you took, once you get through the next door, the old man and his nurse will be waiting. He'll compliment your skills, mention you'll meet again soon, and then teleport you back to the hospital, allowing the game to continue as normal.
Now you're in the hospital lobby. Unfortunately, the main entrance is blocked by a cave-in, so you'll need to find another way out through the west wing. The catch? That door is locked. Right in front of the west wing entrance, you'll find a Pixie. She's looking to escape to Yoyogi Park and will join your party until you get there. She'll tell you that a Gaki has the keycard you need for the west wing. The Gaki are holed up in a locked room on the second floor. Lead Pixie to the door, and she'll call out to the Gaki, offering them a bunch of Magatsuhi. The Gaki will open the door, but when Pixie tells them to hand over the card, they get angry about being tricked and attack!
