Alright, so you've made it to the underpass entrance of the hospital, and guess what? A giant, mutated leech is blocking your path. This thing is basically the queen mother of all the smaller leeches you'll find around. It apparently escaped from Birkin's lab and decided this underpass was its new home, spawning little leeches that are slowly trying to take over the place.

When you first see it, the Giant Leech is usually hibernating after spawning its offspring. But if you wake it up, it gets super aggressive and will definitely try to take you down. It moves by slithering along the ground, and when it gets close, it'll flip up and smash down on you. Unlike its smaller kin, this big guy isn't all that interested in just draining your blood; it's more about direct attacks.

Watch out, because the Giant Leech can spit a nasty green acidic mist. It can also shoot out tentacle-like things from its mouth to knock you off your feet. You can spot where these tentacles will pop out from the water by a small discolored patch. It usually moves pretty slowly, but when it decides to get aggressive, it picks up the pace. You'll know you've hit it successfully because you'll see blood and the controller will vibrate.

Strategy:

• The Giant Leech is particularly weak against fire and explosive attacks. If you see any valve handles or explosive items nearby, use them! Hitting these objects when the leech is close will deal significant damage. Just be careful not to be too close yourself when they detonate, or you'll take damage too.
• Guns work, but you need to be at close to medium range to inflict damage.
• If you're out of ammo or going for a no-weapon bonus, there's a clever way to take it down, even if your teammates have all died. Lure the Giant Leech to the back of the underpass entrance, near where the speedboat used to be. Get onto the surface of the water and start stomping or slashing it with a knife from the side. The Giant Leech can't change direction easily, nor can it attack sideways or get onto the surface, so it will eventually die from your repeated attacks. This method actually takes fewer hits than dealing with a regular zombie!
• For the no-weapon tactic, keep about 3-5 meters between you and the leech. It has a 'Head Butt' attack, but it needs some space to execute. If you stay at this distance, it shouldn't be able to hit you.
