To use your Wrathful Invoker in Diablo 4, find a Wrathful Outgrowth in a Malignant Tunnel. There are a few methods to get Wrathful Hearts, though they are somewhat luck-based. These methods include Uncertain Hearts, Wrathful Invokers, and random drops from the Echo of Varshan.
Uncertain Hearts
Uncertain Hearts are a gamble in Season 1, similar to Obol gambling. You can spend 20 of each Ichor type (Brutal, Devious, Vicious) to receive a random Malignant Heart from the pool of 32. However, these hearts are weighted, and some rare ones only drop in World Tier 3 or World Tier 4. It's recommended to save these rolls until you reach World Tier 3 after beating the Cathedral of Light Capstone Dungeon to increase your chances of obtaining WT3-specific hearts.
Wrathful Invokers
You can also try your luck by creating a random Invoker at the research bench. Choosing the Uncertain Invoker option costs 40 of each Ichor type and yields a random Invoker. If you acquire a Wrathful Invoker, you can use it in Malignant Tunnels. Look for the optional room with Malignant Outgrowths and find the Wrathful Outgrowth. Each tunnel may contain either a Monstrous Outgrowth (to spawn the seasonal boss) or two random heart types. If you find a Wrathful Outgrowth, use your Wrathful Malignant Invoker on it to spawn a Malignant Foe. Defeating this foe may yield a Caged Heart, which contains a random Wrathful Heart (either class-specific or for all classes, depending on your World Tier).
Echo of Varshan
The Echo of Varshan boss fight is another method. Malignant Tunnels can sometimes contain Monstrous Outgrowths that require an Invoker of Varshan (WT1/2), a Foul Invoker of Varshan (WT3), or a Tormented Invoker (WT4/Uber Varshan). Defeating the boss rewards loot, Ichors, and a Caged Heart. This Caged Heart can be any type of Heart usable by your class or available on your World Tier. While odds favor Brutal, Devious, and Vicious hearts, it's still a viable way to obtain Wrathfuls.
The two known Malignant Tunnels that host the Echo of Varshan fight are:
- The Boiling Wounds - Eastern Fractured Peaks
- The Ravening Pit - Untamed Scarps of the Dry Steppes, near The Ruins of Qara-Yisu
