Here’s a map showing you where you can find all the Diablo 4 Extremely Rare Spawns, for your Trophy Hunter achievement, and your fancy special gear.
Below we will go through the rare spawns in each region. Also, some of these creatures are part of a Trophy Hunter in-game challenge. So, you can use this list to help you through the challenge. Note that you only need 15 to get the Trophy Hunter achievement in Diablo 4. The 15 you need for the Trophy Hunter achievement are:

Note that the 15 for the achievement, and the other 5 they placed around the world drop some interesting weapons and items. Bear in mind these items can go Ancestral and Sacred, so, if one of these items is very good for your build, like the Rabies pants, or the poison dagger, then you can repeatedly farm them if you so wish, until something with actual BiS stats drops and you can safely replace them. Kripp, likely the reason why you are here, mentioned about Ancestral and Sacred rolls on these and then enchanting them, so these can serve as both leveling and end game gearing processes if you have a particular stat you want.
