    Start the level, open the door, but do not enter the large room yet. Clear out all enemies (Zombiemen, Shotgun Guys, Imps, Nightmare Spectres) from the entrance.

    Head straight across the room and up the stairs. At the top, turn left and go down the stairs.

    Proceed straight and enter the opening to your right. Lower the lift, ride it to the top, and follow the pathway to an opening.

    Go through the opening. Kill the Shotgun Guys, collect Medikits and Shotguns, and grab the Yellow Key.

    Turn right. You will see a Yellow Door to the left of the stairs. Clear out the Zombiemen and Shotgun Guys, then open and enter the Yellow Door.

    Turn left. Run into the slime pit. Down here, get on the platform, grab the Medikit, go through the opening, and grab the Backpack. A wall to your left will lower; go through it.

    Head back up the stairs. Walk past the pillar. If on Ultra Violence, prepare for your first Revenant. Go through the opening to your right and kill all enemies, including the Revenant.

    Head straight to another door. Before opening it, turn around, kill the Imp to the left, and grab the Medikit.

    Turn back and open the door. Kill all Zombiemen, Shotgun Guys, and Imps. Walk up to the silver wall in the corner; it will lower. Ride it to the top.

    Follow the hallway to find the Red Key. Kill the Demons and Shotgun Guys along the way. Pick up the Red Key and the Medikit.

    Return to the lift and go back down. Exit this room.

    Follow the left-hand wall (avoiding openings to your left) until you reach another Yellow Door, killing any Zombiemen, Shotgun Guys, or Imps.

    Open and enter the Yellow Door. Head right until you see the Red Door to the left.

    Open and enter the Red Door. Kill the Imps and grab the Blue Key and Medikits.

    Turn around and leave this room. Head right back to the Yellow Door, then open and go through it.

    Go up the stairs, killing Zombiemen. At the top, go through the opening and follow the hall to a dark room, killing Imps, Zombiemen, and Shotgun Guys. On Ultra Violence, kill the Revenant here as well.

    Hit the switch in the dark room. Turn around and follow the hall back to the opening. Turn right.

    Kill the Zombiemen, Shotgun Guys, and Imps in front of you, then enter the new opening.

    Kill the remaining enemies. Open the exit door to your left and go through it.

    Grab the Medikit, kill the Imps, and hit the exit switch.
