The Letho of Gulet boss fight is best approached aggressively. With maxed Hunter’s Armor, you should be tanky enough to avoid being one-shot. Spam Aard to keep him off balance and attack. Aim to defeat him within 30 seconds. If Vigor runs out or he casts Quen, roll around him without giving too much ground, as he will start throwing Bombs and Signs. Maxed Footwork allows safe evasion of his attacks while he has Quen active.
The Kayran boss fight is a major challenge. Drink Mongoose and optionally Rook potions. Apply a Whetstone to your Silver Sword. Do not use Swallow, as the Kayran's attacks deal 45% health damage without Quen, making reloading a save more efficient than waiting for health regeneration. The Kayran can be unpredictable, and two hits will likely result in death, as no Act I armor, potion, or perk can withstand two hits.
In the game’s Epilogue, when you reach Letho, you will again have a choice: let Letho go or kill him. After a flashback with the Wild Hunt, tell Letho it is time to finish this and give it his best shot. Killing him will unlock your final achievement for the playthrough.
To unlock the "Ricochet Achievement" and other ability-related achievements, spend at least 6 Talent Points in the Witcher Training Path, saving 2 for Ricochet.
