Alright, so Part 57 is where things get pretty intense and wrap up a major storyline. You'll find yourself on a street, not marked on the map, and your car will conveniently stall out with black smoke pouring from the engine. This leads into a car chase, and depending on how you handle it, you'll get one of two possible final cutscenes.

The Chase: Your main goal here is to chase down the gangsters' car. You can either try to wreck their car yourself by ramming it or pushing it into obstacles, or you can simply follow them closely until their car stops or crashes.

The Confrontation: Once the chase concludes, Tommy will get out of his car. The cutscene begins with Paulie yelling, "Go and nail him!" Tommy aims his handgun at the driver but hesitates. Paulie, less sentimental, shoots the driver, reminding Tommy, "You can't feel sorry for these animals. He'd shoot you in the back the first opportunity he gets."

Paulie's Reload Issue: Paulie then tries to shoot the passenger, but his gun clicks empty. He dismisses it, saying, "This one's finished. One less to worry about."

Tommy's Conscience: During this, Tommy's voice-over to Det. Norman reveals his internal struggle: "I thought about what my mother or Sarah would do if they saw me there."

The Alternate Crash Cutscene: If Tommy's car only manages to follow closely, the gangsters' car will crash into a wall. The passenger's upper body goes through the windshield, and after the horn blares, the driver falls out and crawls. Paulie still yells to "Go and nail him!" and the same sequence of Tommy hesitating, Paulie shooting the driver, and the gun-click for the passenger follows. Tommy's voice-over about his conscience plays here too.

The Final Moments: Regardless of how the chase ends, the rest of the cutscene plays out. Tommy is still looking at the driver, and Paulie prompts him, "Why are you standing there with that look on your face? Remember? Those guys wanted to do your girl yesterday. You better get used to it." As Tommy turns away, his voice-over concludes, "I got used to it."

Alternative Method (by Aknestik):

This method bypasses a direct chase and involves more on-foot action:

At the gas station, drive the Bolt Ace through the red doors. Wait in the car while Paulie handles the first group of enemies.
Drive down a step (carefully, to avoid flipping the car) towards the next group of fighters. Hit a couple with the car, then drive to the fire escape stairs and park.
Go up the fire escape, across the ramp, and drop down to the crates and containers.
The initial gangsters are non-solid and don't trigger a fight, but one further in the alley will shoot. Take him out with Paulie.
Continue past two more non-solid gangsters to reach two by their car. Knock them out with your Baseball bat.
Run back through the alleys and lots, climbing over boxes to get over a wall by a glowing yellow window. This draws out other melee fighters.
Run past them, go back up the fire escape, over the ramp, and down to the shootout area. This triggers the first gunman to speak his line, "Somebody stopped in for a tuning."
The cutscene leading to the chase appears. Run towards the gangster's car, shooting at the back window. The final cutscene will then play.

Note on Aknestik's Method: The guide mentions this method can be buggy, with Paulie sometimes getting killed if he runs ahead.

Tips

Pay attention to the voice-overs; they really flesh out Tommy's character and his internal conflict.
