Return to the tower where the Brain of Mensis was located. Drop down to find the Blood Rock, the final upgrade item.

Exit the other side of the tower to reach the bridge where the Romarinus Hunter NPC was.

Turn right and descend two staircases. Instead of heading for the lift, look for a cage on the right side that leads down into the abyss.

Find the Brain of Mensis in the darkness. Use the “Make Contact” gesture in front of it and wait for about a minute to receive the Moon Rune.

You can choose to kill the Brain of Mensis for a Living String, which will unlock this item in the Messenger Shops back at Hunter’s Dream.

Take the elevator to the dark abyss.

Return to Mergo’s Loft: Middle. Follow the Shadow of Yharnam enemies and then head up the steps to defeat the caster.

Ignore the next set of stairs and take the left path to an alcove containing Blood Stone Chunk x2.

Head back up the stairs and visceral attack the Maneater Boar.

Continue past the items to kill two more Maneater Boars that approach from the right.

Turn around and find a set of stairs heading down. This leads to two more Shadow of Yharnam, Blood Stone Chunk x2, and a Wandering Nightmare.

Return to the grand staircase and take the stairs down the other side. Open the chest to find an Iosefka’s Blood Vial and unlock a shortcut before approaching the lift to the boss.
