Alright, so you've made it to the Vileblood Queen's Chamber and found the Unopened Summons sitting on a table. This is a pretty significant item because it ties directly into what happens with Annalise, the Queen of the Vilebloods, and her loyal hunter, Alfred.

Basically, if you decide to give this Unopened Summons to Alfred, he'll go on a bit of a rampage and kill Annalise. It's a pretty grim turn of events, but it's one of the key questlines in the game. After he's done his business, you can go back to her throne chair and pick up the Queenly Flesh. Don't toss that item aside – it's important later!

You'll need to take that Queenly Flesh to the Altar of Despair, which is located in the Upper Cathedral Ward. Just a heads-up, you can only do this after you've defeated Ebrietas, Daughter of the Cosmos. If you manage to bring the Queenly Flesh there, you can actually revive Annalise. It's a pretty wild bit of lore and a neat way to interact with the world.
