Chapter 2: Remnants

1. Document #3: Lakeside town note — Found in the lighthouse area, outside the first house on the right, by a fire on a beam.

Chapter 4: The Patient

1. Document #4: Villager's note — In the first house on the right from the chapter start. Squeeze through a gap in a corner into another room; the document is on the bed.

Chapter 5: Inner Recesses

1. Document #5: Underground facility note — In the blue tile puzzle room, specifically the second room entered. Found on a table in the back corner.

Chapter 6: Losing Grip on Ourselves

1. Document #6: Graveyard note — After the dual Ogre fight, located in front of horse statues on the ground steps.
2. Document #7: Cemetery lab note #1 — Found within the make-shift lab after the body puzzle area.
3. Document #8: Cemetery lab note #2 — In the room following the lab, found by a body.

Chapter 7: The Keeper

1. Document #9: Catacomb note — After obtaining the first old lithograph key item, a wall opens revealing a living area. Check the table to the right.

Chapter 8: A Planted Seed Will Grow

1. Document #10: Cave note — Found on the floor in a narrow area where you must place a plate into a door to proceed.
2. Document #11: Research facility note — Near the end of the chapter, in the hospital. In a circular room after a scene, check equipment to the left.

Chapter 9: The Cruelest Intentions

1. Document #12: Mansion dining room note — Upon entering the mansion, found in the dining room (left door next to the save room) on the table.
2. Document #13: Ruvik's room note — Inside the mansion, upstairs via the left stairs. Enter the left door, then the first door on the left, and check the desk.
3. Document #14: Mansion music room note — Inside the mansion, through the right door behind the stairs. Found under a painting in the music room.

1. Document #15: Mansion basement note — Found outside the first save room at the start of the chapter.

1. Personal Document #14: Note from Myra — Located near the subway bathrooms at the start of the chapter, inside the red save room.
