Explore Part 19 of our Binding of Isaac: Rebirth guide, covering items like Monstros Tooth, Lemon Mishap, and Mom's Bottle of Pills. Learn their effects and strategies.
Alright, let's dive into Part 19 of our Binding of Isaac: Rebirth item guide. We've got some interesting active items here that can really change up how you approach a room, so let's break them down.
First up is an item that feels like a bomb but with a bit more flair. When you use it, a crosshair appears at your feet, and you can move it around with the arrow keys. After a few seconds, a rocket crashes down on that spot, dealing heavy area damage. Just be careful – if you're caught in the blast, you'll take a full heart of damage. Think of it as a portable bomb that you can aim. It's great for boss battles, but you'll want to make sure you've got a clear shot. Missing can be costly, and if you're going to miss, you might as well just use a regular bomb. It's a decent item, but there are other recharge items out there that might offer more bang for your buck.

| When used a crosshair is placed on the ground beneath your feet. It can be moved using the arrow keys. It will begin to flicker quickly and after a few seconds a rocket will crash into the position. This does heavy area damage similar to that of a bomb. If you're caught in the blast you'll take a full heart of damage. | After the novelty of this item wears off, what are you left with? Actually something fairly decent. In truth, this item's just a bomb that can be used anywhere in the room and can be moved around as it homes in. I'd suggest using this for boss battles, but you probably shouldn't be in dire need of it, especially considering you can miss fairly easily. This means you're just going to wait until you can get a sure shot and then why don't you just use a bomb? A decent item, but there are more valuable recharge items to be had. |
Next, we have the Lemon Mishap. When you use this, a puddle appears around you, and any enemy that steps in it takes damage. It's a pretty clever item, but it has its downsides. Flying enemies won't be affected, and enemies that can shoot from a distance don't need to get close to your puddle. However, for most ground-based enemies, it can deal some decent damage. If you place it strategically, it can even act as a sort of shield. You'll likely still need to dodge projectiles, which might pull you away from the puddle, so keep that in mind. It's a solid item, especially with its short 2-room recharge time.
| TABLE POSITION | ROOM RECHARGE |
| Row 7, Column 3 | 2 |
| EFFECT: When used a puddle of lemonade appears around the player (or possibly urine). If an enemy steps in the puddle, it takes damage. | |
| COMMENTS: I must admit, this item can be used very cunningly. Unfortunately though, flying units are unaffected and units that can shoot don't need to get close to you. But all other enemies can be dealt some decent damage and if placed in a well thought out spot, can create a really effective shield and safeguard for you. You'll more than likely have to dodge projectiles though, which quite often takes you away from the puddle. Keep this all in mind when you're placing it. It's a relatively good item though, plus only a two recharge. | |
Then there's Mom's Bottle of Pills. This one gives you a random pill when used. Now, this item shines when you've got certain other items. If you have Cain's starting Lucky Foot or the PHD, the pills you get are very likely to be stat ups or have positive effects. I've seen players max out stats using Cain with this item, and maxed stats are a huge advantage. Even with the 6-room recharge, if you clear every room on each floor, you can get a good number of pills, leading to a lot of stat boosts. If you don't have those specific items, the Pill Bottle is still useful. Once you've used six pills in a run, their effects will be displayed. You might lose a stat point or two initially, but after that, you can just avoid the bad pills. The benefits usually outweigh the risks. If you get a string of really bad pill effects, you can always ditch the item and look for something else. It's a favorite recharge item for many.
| TABLE POSITION | ROOM RECHARGE |
| Row 11, Column 2 | 6 |
| EFFECT: Gives the character a random pill. | |
| COMMENTS: This item is part of various combos and for good reason. If you have the Lucky Foot (Cain's starting item) or the PHD, all of the pills that you receive from this thing quite often turn out to be stat ups and at the very least, give positive or neutral effects. I've maxed every stat when using Cain with this item and maxed stats is insanely helpful. The six room recharge may seem a little off-putting, but if you complete every room on every floor, you should be able to get 3 or 4 pills on the higher levels. That's a lot of stat ups in a short time! The non-stat up items are few an far between so you're pretty much guaranteed it. If you don't have either of the items which guarantee pills with good effects, the Pill Bottle is still helpful. Once you've consumed all of the six pills for the game, the next time you get them their effect is displayed. Although you may have lost a point or two in stats, it's only once and from then on you can simply not use the bad pills. The benefits are generally quite good and if it turns out a really bad bunch of pill effects are being used for the dungeon, simply throw it away and find some other recharge item. This is one of my favourite recharge items. | |
Finally, we have Monstro's Tooth. When you use this, you summon Monstro himself to slam down on an enemy, dealing massive damage. Just a heads-up: if there are no enemies in the room when you use it, Monstro will land on *you* instead, dealing damage. You also can't use this against Monstro himself. It has a 3-room recharge time.
| TABLE POSITION | ROOM RECHARGE |
| Row 9, Column 6 | 3 |
| EFFECT: When used, Monstro is summoned and slams onto an enemy, dealing high levels of damage. If there are no enemies present in the room when Monstros Tooth is used, Monstro instead lands on you, dealing you damage. This cannot be used against Monstro himself. | |
| VISUAL: Monstro's Tooth hangs on a necklace around the character's neck. | |
