   Location: Room 408 (Restroom). After a cutscene with Pyramid Head, access Room 408 via Rooms 405 and 407. Defeat the Swarm that attacks and pick up the serum from the medicine case on the sink.

   Area: Dr. Fitch's Office
   Location: Examination Room. Enter the room down the dark corridor, examine the locked case on the dresser. After defeating Nurses outside, enter the room near reception and collect the serum from a medical case on a shelf.

   Location: Hell Descent. Before pulling levers for the fans, duck under Fan 3 and Fan 2. The serum is in a medical case on a table behind Fan 2.
