Walkthrough
- 1In the Operation Theatre Balcony, examine the PATIENT'S CHART on the shelf.
- 2Run to the far end of the balcony and step down the stairs to the right.
- 3Move along the right side of the walkway, turn off your flashlight, and choose to "Drop Down" at the end where there is no railing.
- 4In the Operation Theatre, ensure your flashlight is off. Listen to the radio static for nearby enemies.
- 5Turn around to face a Nurse near the operating table. There are three Nurses total on the east side of the table. If fighting, use a charged strong attack. You can lure them by standing still with the light off.
- 6Be careful not to bump into objects, as this will alert the Nurses.
- 7Examine the double doors to the Operating Room; they are chained shut.
- 8Turn east, cut the skin wall with your combat knife, and squeeze through the opening.
- 9In the East Staircase area, pick up the CHILD'S DRAWING (2/18) from the wall opposite the skin wall.
- 10Move up the stairs and examine the body on the gurney. Take the ++OPERATING THEATRE KEY++ from its hand.
- 11Go back down the stairs and squeeze through the wall again.
- 12In the Operation Theatre, approach the chained doors to the Operating Room. Press LB when the icon appears, select the Operating Room Key, and use it to unlock the chain.
- 13Open the doors and enter the Operating Room.
- 14Examine the ELECTROSHOCK NOTE on the counter.
- 15Open the door to the hallway. A cutscene will play where you can choose dialogue options with Josh. These choices do not affect the outcome or ending.
- 16Objective Update: Find Joshua's Toy.
- 17Open the door to the left of the gate.
- 18In the Nurse Center, take the HEALTH DRINK from the shelf and save your game at the SAVE POINT.
- 19Examine the MISSING PATIENT BULLETIN on the left side of the controls by the window.
- 20Exit through the south door into the hallway.
- 21Enter Room 203, the only door that can be opened.
- 22Walk toward the bloody mirror and examine it. Keep tapping the button prompt to break free from the grip and retrieve the ++ROBBIE TOY++.
- 23Objective Update: Return the toy to Joshua.
- 24A Swarm will emerge from an incubator after you take the toy; be prepared to fight them.
- 25Pick up the CHILD'S DRAWING (3/18) from the wall to the right of the mirror.
- 26Return to the Hallway Outside Operating Room via the east end of the Nurse Center, saving your game along the way.
- 27Approach Josh at the gate, open your inventory, select the Robbie Toy, and give it to him. A cutscene will play.
- 28Walk through the now open gate and pick up the CHILD'S DRAWING (4/18).
- 29Most doors down the hall are locked. Enter Room 206, which is open. The door will shut as you approach it.
- 30Take the CHILD'S DRAWING (5/18) from the front of the Room 206 door.
- 31Continue down the hall to the elevator. Press the call button on the left wall to trigger a cutscene.
Tips
- If you look off the side without railing in the Operation Theatre Balcony, you can see Nurses below.
- You can lure Nurses around the room and make them stop if Alex stands still with the light off.
- Be careful moving around in the Operation Theatre; bumping into objects will alert Nurses.
- During the transition to the next area after the elevator cutscene, Alex's health will be fully recovered. This is a good opportunity to take advantage of it, especially on hard mode.
