Walkthrough for Vampyr's "Take Me to a Hospital" quest, including choices with Clay Cox, finding Milton's Shotgun, and more.

Walkthrough
  1. 1
When you first arrive, Dr. Swansea and Nurse Crane will be blocking the steps; go left.
  2. 2
You will see two men fighting ahead, one of whom is stabbed, triggering a blood frenzy. Walk forward to meet Clay Cox.
  3. 3
Answer Clay Cox however you please. You will then receive a tutorial on mesmerising citizens (using a skill to influence an NPC (non-player character — anyone the game controls, not you)). Do so and lead Cox into the storeroom out of the way.
  4. 4
You will be presented with a choice: "Embrace" or "Release".
  5. 5
Embracing Cox: Earns you 500XP (half of his potential), 50/-, his wedding ring, and Clay's Hatchet (an indifferent melee weapon). This also unlocks the "Just take a bite" trophy.
  6. 6
Releasing Cox: Gives you the option of killing him later and unlocks the "Merciful release" trophy. This choice also does not invalidate a pacifist playthrough.
  7. 7
Regardless of your choice, search two containers in the storeroom for Spring (x2) and Tiny Common Handle (x2).
  8. 8
Head back out onto the wharf and proceed towards the steps. Loot another crate before going up.
  9. 9
You will emerge by some tents. If you wish to explore, head northeast along the street. Ignore the bridge on your left (it leads to Whitechapel, which you should avoid for now) and continue northeast into a blind alley.
  10. 10
Enter the building and examine the container just inside the door for a Lead Rod and a Lead Plate. Head up the stairs and search the container by the locked door for a Watch and 5/-.
  11. 11
Return to the street and go through the back gates of the hospital. You will find Rakesh Chadana, who operates a makeshift morgue.
  12. 12
To unlock a hint about Rakesh, select "Your life in London" / "Not afraid of the dead" / "Most people fear dead flesh". If you then follow the "Personal questions" line, you will unlock a new conversation thread: "You believe in life after death?".
  13. 13
You can trade with Rakesh. Do not buy crafting recipes from him, as you will find them later.
  14. 14
To the right of this area is a fenced-off enclosure with several dead bodies. Search them for a Crumpled Letter, which provides a hint about Milton Hooks. Search another corpse for 5/-.
  15. 15
Back on the street, there is an alleyway leading into the Whitechapel District. You can explore it for minor loot, but turn back if you encounter enemies, as they are too tough at this point.
  16. 16
Back in the Pembroke Hospital district, look for 5/- on a barrel by a shop window.
  17. 17
At the end of the alley, go through a gate into the hospital grounds. Search a crate for some ammo. Continue further down to fight three Skal enemies, which are similar to William Bishop but easier to kill.
  18. 18
At the point marked on the map, you will find a quest item, Photography in a Wallet, which initiates the investigation "Midnight in the garden of good and evil".
  19. 19
Return to the hospital's front entrance to find Dorothy Crane.
  20. 20
If Clay Cox survived your meeting, tell Dorothy about him. You can ask her general questions, and she will direct you to your office.
  21. 21
Before heading to your office, you can introduce yourself to other people. You will likely find Gwyneth Branagan in one of the tents.
  22. 22
To unlock a hint about Corcoran Tippets, select "Your life in London?" / "Tell me about the staff" / "Anyone that stands out?".
  23. 23
You will also find Corcoran Tippets tending to the sick.
  24. 24
To unlock hints about Corcoran and Gwyneth, select "Your life in London?" / "Tell me about the staff" / "Positive characters?". If you spoke to Gwyneth, you will have a conversation option: "You are exhausted". If you find Gwyneth Branagan again, you will have a new conversation option under "Personal Questions": "Tippets praises you".
  25. 25
Milton Hooks is standing near the entrance.
  26. 26
If you found his wallet earlier, return it to him for 160XP, 40/-, and hints about him and Pippa Hawkins. If you explore the "Personal Questions" tree, you should have two blue options: "You're extorting money" and "Your relationship with Nurse Hawkins?".
  27. 27
You can also trade with Milton. The outstanding item is Milton's Shotgun (3/11). You can get this at a discount later if the area reaches Sanitized status, or obtain the ultimate discount by embracing him when the time comes. Note that Milton's Shotgun is a double-barrelled gun, meaning its apparently higher per-shot damage over the Lupara is for two shells, not one.
  28. 28
Enter the hospital through the front door. In the room on the right, you will meet several patients. The first is Thelma Howcroft, who claims to be a vampire.
  29. 29
To unlock a hint about Thelma, follow the line of questioning "Your life in London" / "You are a vampire?" / "Ever heard of Cotard's Syndrome?". Select "Are you a patient here?" / "We'll take care of you" to unlock the "Wrong Target" investigation.
  30. 30
Search the table next to Thelma's bed for a piece of lore: About the use of garlic and wooden stakes (3/30).
  31. 31
The second patient is Thomas Elwood, who has terrible injuries from the war.
  32. 32
Having spoken to Thelma, select "Personal questions" / "About Miss Howcroft's disease?" to unlock a hint about Thomas. Speaking to him will unlock a new conversation option for Thelma: "Tell me about Thomas Elwood?".
  33. 33
You can also speak to Sean Hampton, but you will not unlock any hints about anyone.
  34. 34
Head across the wharf and go left to avoid Dr. Swansea and Nurse Crane blocking the steps.
Tips
  • If you are playing on PC or PS4, consider backing up your save before killing Clay Cox to unlock the "Just take a bite" trophy. This is to avoid a potential third playthrough if you are aiming for a high-mayhem playthrough.
  • Milton refreshes his stock daily. Consider buying Common Handle Parts and Trigger Parts to upgrade your weapons. It is also worth buying Bullets and Shotgun Cartridges from him, as there is a finite number of these items available in the game.
  • The alleyway into the Whitechapel District contains tough enemies; turn back when you see them.
  • Choice of: 500XP, 50/-, wedding ring, Clay's Hatchet (if Embracing Cox) OR option to kill later, "Merciful release" trophy (if Releasing Cox).
  • Spring (x2), Tiny Common Handle (x2).
  • Lead Rod, Lead Plate.
  • Watch, 5/-.
  • Crumpled Letter (hint for Milton Hooks).
  • 5/-.
  • Photography in a Wallet (initiates "Midnight in the garden of good and evil" investigation).
  • 160XP, 40/-, hints about Milton Hooks and Pippa Hawkins, Milton's Shotgun (if returned wallet to Milton Hooks).
  • About the use of garlic and wooden stakes (lore).
