Pathologic 3: Quarantine received mixed reactions upon release. Players and critics noted concerns regarding its writing and overly "gameified" mechanics. While appreciated as an introduction to the Bachelor's reimagined story, some players expressed reservations about the narrative direction and characterization, as well as the emphasis on precise diagnosis and interpretation of events. Many believe that the inherent uncertainty is a core element of the Pathologic experience, even when deciphering the plague's specifics.

Rock Paper Shotgun acknowledged the game's success in capturing the spirit of Pathologic by altering the expected gameplay and narrative, stating it was "flavourful and stacked with familiar names and faces for long-suffering fans." However, they also described it as "something of a disjointed mess" as a demonstration of what players would do.

AdventureGamers remarked that Quarantine effectively conveyed "a bleak picture of suffering, dark medicine, and the struggle to keep your humanity when faced with horrible odds." They pointed out that the experimental approach to gameplay, focusing on "what works best" rather than straightforward medical understanding, "goes beyond the perplexities of an unreliable narrator" and can compromise the player's experience if they are unsure of their objectives.

Quarantine was available on Steam until October 12, 2025, when it was delisted and replaced by a Pathologic 3 demo for the full release in 2026. A downloadable version is planned to be made available via the Pathologic Wiki.
