Enforcer: Every time a crime is detected by ctOS head to the search area with the blue triangle in the middle. Once inside the search area take cover and use the Profiler to identify either the victim or the criminal. Wait for the crime to begin before you intervene and save the victim.
Use the Crime Detection System to take down 20 confirmed criminals.

Hard Crash: Performing vehicle take downs are fairly easy and will likely be completed as your progress through the game and takedown either cop cars trailing you or complete Interception Fixer Contracts.
Perform 10 vehicle take downs.
Communication Fail: When performing illegal actions in Chicago civilians will sometimes report you to the police. If they complete the call your reputation will decrease and you’ll need to escape the police scans, so stopping them is important. While there are a diverse way of dealing with them, using Jam Coms will drop the call.
Using non-lethal takedown, stop 10 civilians from calling to report you.
Bookmarked: When entering a restricted area you can mark threats by scoping out the area with security cameras. An enemy is marked when the rank of the threat is placed in red on the screen, such as a star for Elite enemies.
Tag 100 enemies.

Participate in 5 different City Games.
Magic Smoke: Using Focus is best done when you get stuck in a jam and are surrounded. You can either enter a restricted area and gain the attention of multiple enemies and then trigger Focus or achieve this near the end of the game in No Turning Back, Part 2.
Kill 4 enemies within a single instance of Focus.
Escape Loop & Free Radical: Both of these achievements require you to escape police chases. The only difference between them is that you need to have the maximum Heat level for Free Radical. This means that the police will be intense, with a helicopter on your tail, so use Blackout to get away easier.
Escape Loop: Escape 15 police chases.
Free Radical: Escape a level 5 police chase.
White Rabbit Object & Scanproof: Unlike the two previous achievements in which you must escape the police, this achievement requires you to escape the police scans, one of which is a level 5 scan. These can be triggered by having civilians call the police or naturally as you progress through the game.
White Rabbit Object: Escape 15 police scans.
Scanproof: Escape a level 5 police scan.
Hardware Fail: Shooting out tires seems like something you’ll have to try and complete on purpose, but you will likely get this while completing the Criminal Convoys when the enemies hide behind cars. The other option is to simply find a parking lot or garage and shoot out the tires on the many cars parked there.
Shoot out a tire on 15 different vehicles.
Black Hat Trick: Though it may seem hard to kill three enemies with a single IED, cars in Criminal Convoys will usually carry 2-3 people. This means that if you set an IED in the path of the convoy and trigger it at the right time you can easily complete this achievement.
