You've always got the option to refuse a Flaw, even on Supernova difficulty, so don't worry about being stuck with something you hate. However, there's a trophy called 'Flawed Hero' that requires you to accept three Flaws. If you want the trophy but not the permanent disability, just remember you can reload a save after unlocking it.

Some Flaws, like the 'Addiction' types, will keep hitting you with their penalty unless you satisfy the craving when withdrawal kicks in. Others, like the '...phobia' types, only activate when you're around specific enemies. For example, Robophobia will trigger if you're near automechanicals, and this also applies when SAM is with you.
