Walkthrough

At the start, take the path to the left. After jumping across the water a couple of times, you'll arrive at a bridged area. Walk off the ledge to the right, landing on a ledge from which you can hop over and claim the shotgun. The Lethal and Loaded Trophy should appear immediately. It is recommended to Quit and Reload your game from one of your Save files after earning this Trophy.

Return to the start of the level and take a right at the river, going down the stone steps. Kill the two wolves on your way down.

Climb over a rock, touch the ground, then backflip back onto it. When the wolf you just triggered comes after you, take it out from your elevated position.

Take a right at the tall rocks and slide down the short ramp backwards. Walk a step forward, then immediately jump back up to safety to shoot the three wolves that are triggered.

Return to the tall rock cliffs, climb to the top, and claim the large MediPack.

Make your way down the rocks, but do not drop to the ground yet. Touching the ground will trigger two raptors. To avoid damage and potentially earn the Clever Girl Trophy, go to your far right as you face the clearing, turn around, slide down the ramp, walk forward a step or two, and jump back up to safety. You can then shoot the raptors. If they don't come near you, drop to the ground and backhop carefully.

When the raptors are dead, follow the left wall until you see a small waterfall.

Run over to a small cave opening across the valley, just to the left of the other waterfall, triggering the T-Rex. When safely inside the cave, roll and fire. Avoid getting too close to the opening as the T-Rex can reach into the cave. Patience is key to surviving this battle.

Tips

This level has 5 Secrets, 16 pickups, and 13 kills.
You can earn the Lethal and Loaded Trophy by acquiring the Shotgun weapon at the beginning of the level.
There are 2 Trophies related to the T-Rex: Raid Not Kill (complete the level without firing at the T-Rex) and T-Rextinct (defeat the T-Rex). You must choose one to earn per playthrough.
If you are playing the Remastered game, you can earn the Clever Girl Trophy by avoiding all damage from the raptors in this level and the next one.
To earn the Clever Girl Trophy, after triggering the raptors, slide down the ramp, walk forward a step or two, and jump back up to safety to shoot them.
To earn the Raid Not Kill Trophy, run to the safe cave opening across the valley and fire at the T-Rex from there without hitting it.
To earn the T-Rextinct Trophy, defeat the T-Rex. This allows you to count the kill on this playthrough, keeping the door open for earning the Leave Them Sucking Wind Trophy on this run.
If you had continued after claiming the shotgun at the beginning, you would have sacrificed a protected position for defeating the wolves.
To trigger the wolves (4-6) and fight them from safety, slide down the ramp backwards, walk a step forward, then immediately jump back up to safety and shoot them from above.
You can earn the Lethal and Loaded Trophy by acquiring the Shotgun weapon at the beginning of the level instead of the end.
Make sure to begin by creating the recommended THREE Save files at the beginning of this level.
The T-Rex can get into the recess you are currently in, but a small cave opening across the valley is safe.
The T-Rex CAN get his head partway into the cave and put a major hurt on you.
You get 2 Trophies in this level.
You get the Raid Not Kill Trophy for completing this level without ever firing at the T-Rex.
You get the T-Rextinct Trophy for defeating the T-Rex.
You can't get both Trophies in one pass.
You're going to have to choose to earn one now, and then come back later to earn the other one.
Hence the multiple Save files I recommended at the start of this level.
Since we're doing the No Medi Pack thing on this run, my instinct is to defeat T-Rex now and then come back for the other Trophy later.
It also allows you to count the T-Rex kill on this playthrough, keeping the door open for earning the Leave Them Sucking Wind Trophy on this run.
There are 3 Save Crystals in this level.
Raptors are going to be your biggest headaches here.
You squandered a few shotgun shells on the ones that you had to fight out in the open.
Even so, you finished the level with 38 shells.
Follow the guide for killing enemies from safe spots and be patient, and this level will be relatively easy for you.
Before killing the wolves, have Lara jump from the high cliff to the floor below and die from the fall.
Let the raptors kill Lara.
Let Lara get mauled to death by the T-Rex.
