Location: You get this by defeating the Mad Pumpkin Head enemy at the Foot of the Four Belfries Site of Grace. Head north from there, past the jellyfish, and you'll find an enemy camp. The Mad Pumpkin Head you need to fight is behind a hill. To increase your chances of it dropping, use a Silver Pickled Fowl Foot. This might take a few tries, but if you're a fan of Flails and Strength builds, it's definitely worth it.
