Spine is a creature found in the darkened Apartment 31, specifically in the living room of a bloodsoaked, false version of the neighboring apartment. Spine appears after the player passes back through this area, having accessed the darkened telescope room.

Status: Spine was one of the unwitting neighbors remade by the Stargazer before the arrival of the visitor. She resides in the Stargazer's apartment but has been observing Sam, entering his apartment to be near him and sleeping alongside him. Her presence is foreshadowed by sensations of being brushed against, feeling warm breath, or a clammy hand on the back while in Sam's apartment. Spine desires to remake Sam into her own image so they can be together in her home, away from the bright light. She can access Sam's home undetected by inhabiting a ruined, alternate version of Apartment 33, accessible through Apartment 31 after turning off the lights.

Recruitment: If Spine's HP is reduced to half or less during combat, she will conclude Sam needs more time and can be spared to leave the fight peacefully. After sparing her, the player may receive an event noticing her presence when going to sleep each night, with choices affecting her disposition. She can also be visited in her room for conversation on subsequent days. Spine has a hidden relationship score with Sam, increased by visiting her and being kind during night events. If this score is high enough and Sam breaks through the flesh blocking her front door to reach the Meat World, Spine will temporarily join him as a party member if he has no other companions. Her class is Unraveled, and she starts at level 5.

Battle: Spine attacks by biting, dealing piercing damage and potentially applying bleeding. She can also perform a rapid series of bites. She is resistant to crushing damage and weak to slashing and cold damage, being immune to flesh damage. She has a 30% resistance to paralysis and stun.
