This guide covers the basic controls and mechanics for playing Car Mechanic Simulator 2021. Understanding these fundamentals will help you perform repairs efficiently and achieve better results.

Controls:

Fingers: A, W, E, R, Spacebar
Move Hand: Move Mouse
Move Hand Down: Mouse Click 1
Rotate Hand Left/Right: Mouse Click 2 + Move Mouse

You can either move your hand or rotate it, but not both simultaneously. This can be challenging when performing delicate tasks. All directional references (left/right) are from your perspective.

The default hand position is parallel to the floor, with fingers uncurled and the hand raised.

Picking Up Objects:

Large Objects: Position the object below the center of your hand. Hold Mouse Click 1 until you are touching the object, then press and hold A, W, E, R, and Spacebar.
Knives: Position the knife between your forefinger and thumb. Hold Mouse Click 1 until you are touching the object, then press and hold A, W, E, R, and Spacebar.

Manipulating Objects: Rotating your hand slightly to the left from the default position can make manipulating tools like the circular saw or laser easier.

Stabilizing Blood Loss:

To stop bleeding, pick up the green needle and stab the patient.
If you pick up the green needle from directly above, you will prick yourself, causing hallucinations. To stop hallucinations, prick yourself with the blue needle. Be aware that the blue needle is poisonous to the patient.

Preparation:

Patient bleeding only starts when you begin cutting. You can clear unnecessary tools from the table before starting.

Achieving A++ Rank:

This requires speed, accuracy in cutting (to minimize blood loss), and maintaining blood levels. It is recommended to attempt A++ ranks after completing the game.

- Heart Surgery

Walkthrough

Use the hand circular saw to detach the ribcage. Touch the outer portion of the ribcage with the tool.

Pick up and remove the ribcage.

Pick up and remove the left lung by grabbing it and moving your hand left and right.

Pick up and remove the right lung by grabbing it and moving your hand left and right.

Pick up a knife and cut the upper portion of the pink organ and the bottom portion of the pink tubular organ. Remove it.

Cut the upper heart valves with the knife until the heart stops beating.

Remove the old heart.

Open the heart container located on the right side of the table.

Place the new heart into the area where the old heart was.

Tips

For an A++ rating, perform all steps accurately and quickly. Avoid hitting unintended parts.
When using the saw, lightly touch all parts of one rib.
Cut only the necessary parts.
Before placing the new heart, stabilize the patient.

- Kidney Surgery

Cut the dark, large intestine on the lower right side and then the less obvious left-middle side. Remove these parts.

Cut the stomach from the top tube and the bottom tube.

Cut the light, pink intestine on the bottom left crimp. Remove the stomach and the intestine.

Remove all organs blocking the kidneys by grabbing and moving your hand left and right.

Cut the tubes connecting both kidneys. Remove the kidneys.

Open the new kidney box and place the new kidneys in the old kidney's spots.

Tips

The left kidney can be difficult to remove by grabbing; ensure your hand is rotated and in the correct area.
Use a pencil to move the kidney around until you can pick it up, as suggested by Grent.
To achieve an A++ Method, perform all steps accurately and quickly. Accurate cutting points should result in zero to little blood loss.
Warning: Stabilize the patient with the green needle if their rate of blood loss is 5ml/second and before you place the new kidneys.

- Brain Surgery

Walkthrough

Use the hand circular saw to cut around the skull to break it up. The cap should fall off and the patient's head should now be limp.

Alternatively, use the black, hand laser (all the way to the right) to break up the skull with no bleeding, being careful to avoid hitting the brain or face.

Remove the skull cap with your hands.

Pull out the brain with your hands to expose the one big nerve.

Stab the nerve with a knife until the brain becomes detached.

Remove the brain by grabbing it and violently moving your hand left and right.

Open the brain jar, pick up the new brain, and place it in the hole without letting go.

A++ Method:

Use the laser tool. Grab the laser with your hand slightly rotated from the default position.
While the laser is warming up, knock open the brain jar's lid.
Use the laser accurately to allow the patient to suffer zero (or close to none) blood loss from the skull cap removal.
Laser half of the cap off with one angle, then flip your hand around and do the other side of the skull.
If you need to flip your hand, do so behind the patient's skull to prevent any misfiring.
The laser box has a big, red emergency shut off button. Hit this as you go grab the green needle.
At most, you should suffer a blood rate loss of 9ml per second.
After removing the skull cap, use the green needle to stop blood loss by stabbing the patient in the face with it.
Pull out the brain and give a few quick jabs with the kidney knife (the big saw thing in the center table).
Quickly install the new brain. Do this fast.

Tips

Avoid hitting the brain or face when using the laser.
Flip your hand behind the patient's skull when rotating the laser angle to prevent misfiring.
Act quickly when performing the steps after removing the skull cap.
